Improving Maine Healthcare Quality

Director of Care Coordination

The Director of Care Coordination serves as a key leader in our population health strategy by providing strategic leadership and operational oversight for our care management program across all settings of care. This person will serve a central role in the continued development and evolution of care transformation work within the St Mary’s system and extending to our community partners with a goal to create a patient centered vs. care setting centered approach to care coordination. Central to this role is creating systems that foster smooth and seamless transitions of our patients across settings of care, and ensuring that we are proactive, intentional, and appropriately standardized about the setting in which the patient is receiving services. As metrics emerge for measuring population health, it will be the responsibility of the Director of Care Coordination to apply and report these measures.
